When should I enable “Hidden SSID”?

If you can see your WiFi name in the phone WiFi list, SSID broadcast is enabled, so keep it off. If your phone cannot find the WiFi name even after refreshing, SSID broadcast is disabled, so you must enable Hidden SSID.

What format is encoded in the QR code?

It uses the common WIFI:... payload format (ZXing spec) supported by most scanners.
